The Ghoulish Gouger is a post-Moon Lord rogue boomerang that is dropped by Polterghast. When used, it throws a set of four ghoulish scythes that go through tiles and return to the player after traveling a moderate distance.
Performing a stealth strike with the Ghoulish Gouger will cause the first scythe thrown to release a circular pattern of 8 tormented souls on enemy hits that will home on nearby enemies a moment after being released.
Its best modifier is Flawless.

- Buffed damage from 110 to 166.
- No longer throws out one ghoulish scythe that grants enemies 2 local immunity frames; stealth strikes no longer spawn waves of souls. When used it now consecutively throws out four ghoulish scythes at very high velocity that grant enemies 10 local immunity frames. Stealth strike now spawns in three tormented souls on the first enemy hit that deal 85% base damage.
