The Primordial Wyrm is a hidden boss battled in the 4th layer of the Abyss. It will spawn at any time if a player is affected with the Chaos State debuff (given by Rod of Discord, Normality Relocator, and similar items) while in any layer of the Abyss.
Its only unique drop is the Halibut Cannon, a powerful endgame gun.

Behavior
The Primordial Wyrm is an aerial worm-like enemy with 42 segments in total. Its body and tail segments can neither take damage nor deal contact damage. It performs a variety of attacks on a cycle that changes slightly with each phase transition, quickly chasing players and attempting to ram them while not using any attacks. When transitioning between phases, the wyrm will choose the next attack in its next phase's cycle. Upon contact with the head, the player is inflicted with the Crush Depth debuff for 10 seconds.
While it can be spawned in any layer of the Abyss, it will enrage outside of the 4th layer. When enraged, the wyrm moves and spins faster, lightning bolts and Ice Mists are more predictive, 1 extra Eidolist and 2 extra Ancient Dooms are summoned in their respective attacks, and it will periodically use a telekinetic attack that yanks the player towards its head, accompanied by a loud sound and a burst of blue sparks.
Phase 1
- Performs the following attacks in a cycle:
- Quickly moves away from the player, roars, and charges at the player for 5 / 4 / 3.5 / 3 seconds, sharply homing in on their position. It charges three times before transitioning into the next attack.
- For 3 seconds, summons three lightning bolts from the sky every 0.5 seconds. One lightning bolt aims ahead of the player, one lightning bolt aims directly at the player, and the other aims behind the player. During this attack, the wyrm itself flies upwards.
- Lightning bolts inflict Electrified for 2 seconds.
- Charges at the player once in a similar manner to the Charge Combo, although with 1.5x the speed.
- Summons an Eidolon Wyrm if there are none alive, accompanied by a sound effect. During this attack, the wyrm itself flies downwards. It transitions into the next attack after 3 / 2.5 / 2.25 / 2 seconds.
- Performs another Charge Combo attack.
- For 3 seconds, summons three homing Ice Mists from below every 1 second. One mist aims ahead of the player, one mist aims directly at the player, and the other aims behind the player. During this attack, the wyrm itself flies downwards.
- Performs another Fast Charge attack.
- Summons 3 / 4 / 5 / 6 Eidolists if there are none alive, accompanied by a sound effect. During this attack, the wyrm itself flies downwards. It transitions into the next attack after 3 / 2.5 / 2.25 / 2 seconds.
Phase 2
- Transitions at 80% health.
- The second charge of the first Charge Combo in the attack cycle is replaced. Instead, the wyrm spins around the player and fires projectiles from its body segments for 5 / 4.5 / 4.25 / 4 seconds before transitioning into the next attack.
- During this attack, the wyrm itself temporarily becomes immune to most slowing or freezing debuffs including Eutrophication, Galvanic Corrosion, Glacial State, Kami Flu, Slow, Temporal Sadness, Time Distortion, Vaporfied, and Webbed.
- 10 main segments simultaneously fire Ancient Light every 2 / 1.83 / 1.75 / 1.67 seconds.
- 20 alternate segments simultaneously fire Shadow Fireball every 1.5 / 1.33 / 1.25 / 1.17 seconds.
Phase 3
- Transitions at 60% health.
- The second charge of the second Charge Combo in the attack cycle is replaced. Instead, the wyrm summons a ring of 4 / 5 / 6 / 7 Ancient Dooms which orbit around the player before bursting into four Prophecy's End in each cardinal direction. After 2 / 1.75 / 1.67 / 1.58 seconds, another ring appears until three rings have appeared in which it will transition into the next attack. During this attack, the wyrm itself flies upwards.
- While performing this attack, the wyrm itself flies upwards and stays invisible.
- The Ancient Doom ring has a radius of 37.5 / 34.38 / 33.44 / 32.5 tiles from the player.
- Ancient Dooms and their projectiles inflict Shadowflame for 2 seconds.
Phase 4
- Transitions at 40% health.
- The final charge of both Charge Combo attacks in the attack cycle are replaced. Instead, the wyrm accelerates towards the player for 3 / 2.5 / 2.25 / 2 seconds. Then, it repositions to the left or right of the player, roars, and charges while summoning a wall of 4 / 6 / 8 / 10 lightning bolts from the direction of the charge towards the player.
Phase 5
- Transitions at 20% health.
- Performs Fast Charge attacks instead of spawning Eidolists and Eidolon Wyrms.
Phase 6
- Transitions at 5% health.
- For the remainder of the fight, it becomes immune to most slowing or freezing debuffs including Eutrophication, Galvanic Corrosion, Glacial State, Kami Flu, Slow, Temporal Sadness, Time Distortion, Vaporfied, and Webbed.
- Replaces all previous attacks with one final attack, combining the elements of the Firing Spin and Ancient Doom Ring attacks. The wyrm itself alternates between flying upwards for a very short time and spinning around the player.
In Expert Mode
- Moves and charges faster with a greater predictiveness.
- Lightning bolts and Ice Mists travel faster.
- Ancient Dooms spawn closer to the player.
- Takes less time to perform most attacks excluding Lightning Rain and Ice Mist Barrage attacks.
- Summons more Eidolists, Ancient Dooms, and lightning bolts during the Lightning Charge attacks.
In Revengeance Mode
- Moves and charges faster with a greater predictiveness.
- Lightning bolts and Ice Mists travel faster.
- Ancient Dooms spawn closer to the player.
- Takes less time to perform most attacks excluding Lightning Rain and Ice Mist Barrage attacks.
- Summons more Eidolists, Ancient Dooms, and lightning bolts during the Lightning Charge attacks.
In Death Mode
- Moves and charges faster with a greater predictiveness.
- Lightning bolts and Ice Mists travel faster.

On the For the worthy seed
- Moves 15% faster.
- Takes half as much time to perform Lightning Rain and Ice Mist Barrage attacks.
- Takes a quarter of the time to perform Eidolon Wyrm Spawn attack.
Notes
- The Primordial Wyrm's awakening by a player depends only on whether or not the player has the Chaos State debuff while in the Abyss, and will therefore spawn if the player enters the biome with the debuff already applied, even if the Rod of Discord or other items which inflict Chaos State are not actually being used within the Abyss.
- As a result, if the player has the Chaos State debuff and is in the Abyss when they kill the Primordial Wyrm, it will immediately respawn.
- The Primordial Wyrm does not require Polterghast to be defeated in order to drop weapons. Although extraordinarily difficult or even impossible to do so, the Wyrm can be defeated and its immensely powerful weapons obtained after obtaining a Rod of Discord at the beginning of Hardmode.
- Enemies spawned by the Primordial Wyrm do not drop items or coins.
- The Primordial Wyrm can only be spawned in Multiplayer by the Chaos State if all players are in the Abyss though it can still spawn naturally.

Trivia
- The Bestiary entry for the Primordial Wyrm: "The first Wyrm, blessed by the Goddess of Life, held staggering power that surpassed nearly all of its auric brethren. It held a deep hatred for any sign of usurped light, and would have attempted to strike down even the wielder of Primordial Light had they returned to reclaim their artifact. And yet, despite its immense power, it never would have stood a chance."
- The Primordial Wyrm's theme is Threats of the Ocean Floor, which was made by the artist DM DOKURO.
- The theme was originally intended to be used for the mini boss encounters in the Abyss before being scrapped and repurposed.
- The sound the Primordial Wyrm makes when summoned resembles the first chord of the theme and was present long before the theme was implemented.
- If the Calamity Music add-on mod is disabled, Boss 3 will play instead.
- Its design is based on a creature from the aquatic exploration game Subnautica, known as the Ghost Leviathan. The original sprite was also a copy of the Ghost Leviathan's design.
- The different life-stages makes this more evident, as there are both juvenile and adult Ghost Leviathans.
- The Adult versions of both creatures share the similar purpose of preventing the player from doing something (Adult Ghost Leviathans protect the crater edge, while the Primordial Wyrm prevents the player from using the Rod of Discord or other short, ranged teleportation items).
- The Primordial Wyrm could be considered the Calamity Mod version of the Dungeon Guardian since both are extremely strong enemies that are intended to prevent the player from performing a certain action.
- In previous versions of the mod, the Primordial Wyrm could spawn at any time in the 3rd and 4th layers of the Abyss before Polterghast had been defeated.
- Similar to the Eidolon Wyrm, it spawns as a passive NPC, and only attack the player if they were affected with the Chaos State debuff, or if it was attacked.
- Prior to the 2.0.2.001 update, the Primordial Wyrm was considered a "Superboss". After the update, its status was demoted to "Hidden Boss", though functionally it was unchanged.
- If the Boss Checklist Mod is installed, its unique despawn message will be "...".
