The Rotten Brain is a Pre-Hardmode accessory that is dropped by The Hive Mind. When equipped, it increases all damage by 10% when the player is below 75% health and reduces the player's movement speed by 5% when they are below 50% health. When the player takes damage, 6-7 shade raindrops fall from the sky which deal a base damage of 18.
