The Toxic Heart is a Hardmode accessory that is dropped by The Plaguebringer Goliath.
When equipped, the player periodically releases a circular "pulse" around them which covers an 18.75-tile radius. The pulse deals 200 typeless damage, affected by the player's highest class damage, and inflicts the Plague debuff onto enemies. The frequency of the pulse increases linearly as the player has lower life regeneration. At 10 or higher life regen, pulses are emitted every 7 seconds. At -30 or lower life regen, pulses are emitted every 0.35 seconds.
Any enemy killed by the pulse will spawn a new pulse, which covers a 12.5-tile radius and deals 80% of the original pulse's damage, and the player will be healed for 10 health. Enemy-spawned pulses will also spawn new pulses if they kill enemies, allowing chaining to occur, with each new pulse dealing 80% of the damage of the pulse that spawned it and healing the player by 10 each time as well.
Additionally, equipping the Toxic Heart increases the damage of player-inflicted sickness debuffs by 1.5x.
