The Amalgamated Brain is a craftable Hardmode accessory and the direct upgrade to the Rotten Brain and Brain of Confusion. It grants the following effects while equipped:
- +10% damage
- While invincibility is active (i.e. when the player takes damage, enters the world, or uses the Magic Mirror or its upgrades), shade raindrops rain from the sky. They deal 60 damage, can pass through tiles, and do not pierce. It also has a chance to inflict the Confused debuff to nearby enemies. The chance to inflict Confused and its duration both scale with the amount of damage the player takes.
- Ability to dodge attacks.
- After dodging, the player is inflicted with the Cerebral Mindtrick debuff, which increases critical strike chance and summon damage by 10%.
- Dodges do not trigger if an attack deals less than 5% of the player's maximum health. Dodge cooldown depends on how much damage was dodged, ranging from 15 seconds at 5% maximum health to 90 seconds at 50% maximum health. This cooldown is shared with all other regular dodges and reflects.

Notes
- Shade raindrops deal magic damage and receive benefits from magic bonuses. However, their damage cannot be increased through any means.
- It can be considered the Corruption counterpart to the Bloody Worm Scarf, even though they both can be crafted regardless of the world evil.
Tips
- If the player is in a Corruption world, the Worm Scarf can be turned into the Brain of Confusion at a Tinkerer's Workshop while in a Graveyard. Conversely, the Bloody Worm Tooth can also be turned into the Rotten Brain at a Tinkerer's Workshop while in a Graveyard.
- While the Bloody Worm Scarf is geared primarily towards melee players, the Amalgamated Brain is geared towards all types of classes, as it increases damage for all classes.
History

- 2.0.4.001: Dodge no longer triggers if an attack deals less than 5% of the player's max health, and now has a varying cooldown based on how much damage was dodged, ranging from 15 seconds at 5% max health to 90 seconds at 50% max health.
- 2.0.1.002: Now inherits Brain of Confusion's dodge effect.

- Buffed base debuff duration from 1.5-5 seconds to 5-8.
- Now uses 3 Souls of Night in its recipe, and now requires an Anvil to craft instead of a Hardmode Anvil.
