The Rare Elemental in a Bottle is a Hardmode accessory that is dropped by Sand Elementals. When equipped, it summons a Rare Sand Elemental that summons healing orbs while the player is damaged. These healing orbs are cast once every two seconds, slowly move upwards before disappearing after a few seconds, and restore 15 health each, effectively granting an extra 7.5 health regeneration per second.

- The effect of the healing orbs and elemental is not boosted by having Summoner gear or a Summon weapon active, allowing the accessory to be used effectively by any class.
- Potions such as Heartreach Potion effectively grant homing to the Rare Sand Elemental's healing orbs.
- The elemental emits a large amount of light when casting an orb.
- The elemental attempts to position itself 10-18 blocks to the left side of the player.
- Equipping the accessory in the vanity slot will summon its minion, but it will not spawn healing orbs.
Trivia
- In the mod's source files, the Rare Elemental in a Bottle is referred to as "WifeinaBottlewithBoobs".
- The Rare Sand Elemental projectile was previously called "Drew's Sandy Waifu" in the mod's source. Drew (also known as MountainDrew or Fabsol) is the lead developer of the Calamity Mod.
- The " ;D " flavor text in the item's description is an emoticon that mimics the item sprite's facial expression.
