Polterghast is a post-Moon Lord boss that can be fought in the Dungeon. While it is optional, it is highly recommended to fight it before challenging The Devourer of Gods.
Defeating Polterghast will provide the player with Ruinous Souls, a material used to craft powerful equipment. Additionally, minibosses in the Abyss will begin dropping weapons upon its defeat, and the final tier of the Acid Rain event will become unlocked.
Summoning
If Polterghast has not yet been defeated in the world, it will spawn after the player kills 30 Phantom Spirits, which can be found in the Dungeon after Moon Lord has been defeated. Its spawn will be forewarned by two status messages: "Wails echo through the dilapidated dungeon halls..." upon killing the 10th Phantom Spirit, then "Long-dead prisoners seek their zealous revenge..." upon killing the 20th Phantom Spirit. Polterghast immediately spawns upon killing the 30th Phantom Spirit.
Alternatively, Polterghast can be summoned manually at any time by using a Necroplasmic Beacon in the Dungeon. After defeating the boss once, this becomes the only way to summon it.
Behavior
Polterghast behaves similarly to Plantera, chasing the player quickly while using four long hooks to grapple on tiles. In the second phase, its hooks will detach and begin firing projectiles independently, before re-attaching in the final phase as Polterghast summons an unbound clone of itself.
If the player is outside of the Dungeon, Polterghast will become enraged, gaining a speed boost. If the player remains on the Surface for over 10 seconds, Polterghast will move away and despawn.
Phase 1
- Polterghast follows a Plantera-like behavior, placing its hooks on nearby tiles to quickly pull itself towards the player.
- Polterghast alternates between moving slowly while firing a spread of 6 slow

Phase 3
- Upon entering Phase 3, Polterghast summons a clone of itself. The clone behaves similar to Polterghast, staying within the hooks and alternating with Polterghast in chasing the player.
- The clone can be killed; however, killing the clone will cause Polterghast to be more aggressive.
- Polterghast's hooks move slower and less often. The chains reappear and the hooks no longer fire phantom shots.
- Polterghast moves significantly faster and deals even more contact damage. Additionally, it fires a spread of 8 blue or red phantom shots every 7 seconds.
- If the clone has been killed, Polterghast will instead fire a spread of 10 blue or red phantom shots.
- While glowing, the clone positions itself on the other side of Polterghast and charges up to 3 times in tandem with Necroplasm.
- If the clone has been killed, Polterghast charges 4 times instead, and its charges are much faster.
- When at 10% / 20%

- Phantom and potent phantom shots will turn around and move towards the player's position after traveling a moderate distance.

Aftermath
- After Polterghast has been defeated, a status message appears in the chat as, "The abyssal spirits have been disturbed."
- New content in the Sulphurous Sea and the Abyss will become unlocked:
- The third tier of the Acid Rain and all of its associated content will become available.
- The Reaper Shark will begin dropping Reaper Teeth, the Valediction, and Deep Sea Dumbbell.
- The Eidolon Wyrm will begin dropping the Eidolic Wail, the Void Edge and the Eidolon Staff.
- The Colossal Squid will begin dropping Calamari's Lament.
- The Bobbit Worm will begin dropping the Bobbit Hook.
- Dungeon Crates will contain Necroplasm, and Hydrothermal Crates will contain Reaper Teeth if The Old Duke has also been defeated.
- The Cyborg will begin selling the Lion Heart.
Notes
- Polterghast moves faster if it is currently inside tiles.
- Polterghast will turn red and the clone will briefly turn invisible when preparing a charge.
Tips
- For more elaborate strategies on defeating Polterghast, including weapon and arena recommendations, see Guide:Polterghast strategies.
- Polterghast has similar AI to Plantera, which means that movement strategies used during that boss fight may work here as well.
- Slowing down and observing how Polterghast behaves shortly before each of its telegraphed dashes would help dodging it, as the boss will only dash diagonally from where it is positioning itself relative to the player.
- For phase 3 especially, dodging towards an axis parallel to where Polterghast and its clone line up would make dodging both a lot easier.
- Don't rush too much when fighting its last form. The chaotic activity can easily overwhelm the player if they're careless. Take your time and be careful when dodging.
- Polterghast can be fought before any post-Moon Lord boss is defeated, although it is highly recommended to defeat Providence and the Rune of Kos bosses first. Polterghast's high speed, damage, and projectiles can easily overwhelm an unprepared player.

- The
Bestiary entry for the Polterghast: "The feral amalgamation that drove out the cult that once dwelled here, and inadvertently broke an alliance thought to be eternal. Many have delved into the dungeon in search of riches and knowledge long since lost, yet most simply join the spirits that have claimed this place as their own."
- Polterghast's theme is Feral Amalgamation, which was made by the artist DM DOKURO.
- Polterghast's theme used to be Ruin, a remix of "In the Hall of the Mountain King" from the game The End is Nigh.
- Polterghast's clone in Phase 3 was inspired by Pontiff Sulyvahn, a boss from the game Dark Souls III.
- The boss changed its name as it entered new phases in earlier versions of the mod.
- The multiple names were the result of a disagreement between one of the boss' spriters, LordMetarex, and the lead developer, Fabsol/MountainDrew. The spriter was naming his files ‘Necroplasm' and was using it as the official name for the boss. During a spoiler, the official name was revealed to be ‘Polterghast'.
